Direct-to-Consumer (Cash-Pay) Model

PHARMACA INTEGRATIVE operates as a cash-pay retail provider. This means we do not directly bill Medicare, Medicaid, or private health insurers at checkout.

Step 2: Obtain Itemized Invoice

When your order ships from PHARMACA INTEGRATIVE, you receive an official invoice containing billing details, model information, and Healthcare Common Procedure Coding System (HCPCS) codes.

Step 3: Complete CMS-1500 Health Insurance Claim Form

Attach our itemized receipt alongside your physician's prescription to a standard CMS-1500 (or your insurer's patient claim form) and mail or upload it to your insurer.
